Abstract

Skin barrier merupakan pertahanan utama kulit yang melindungi dari agresi lingkungan, dan kerusakannya berperan dalam patogenesis berbagai penyakit kulit, termasuk atopic dermatitis (AD). Colloidal oatmeal telah dilaporkan memiliki potensi farmakologis untuk memperbaiki fungsi skin barrier melalui mekanisme multifaset, termasuk peningkatan ekspresi protein struktural keratinosit seperti filaggrin dan claudin-1, serta modulasi biomarker inflamasi dan stres oksidatif seperti IL-1α dan 4-HNE. Penelitian ini menggunakan pendekatan narrative review dengan sumber pustaka dari PubMed, Scopus, ScienceDirect, dan Google Scholar dalam 10 tahun terakhir, mencakup studi in vitro pada keratinosit HaCaT dan uji klinis terkait efek topikal oatmeal. Hasil review menunjukkan bahwa senyawa bioaktif colloidal oatmeal, termasuk avenanthramide, β-glukan, dan lipid, secara signifikan meningkatkan hidrasi kulit, memperkuat tight junction, dan mengurangi transepidermal water loss (TEWL). Avenanthramide menekan produksi sitokin proinflamasi seperti IL-1α dan TNF-α, sementara β-glukan dan lipid mendukung pembentukan natural moisturizing factor (NMF) dan film pelindung epidermis. Uji klinis pada pasien AD menunjukkan peningkatan hidrasi kulit, perbaikan mikrobioma, dan penurunan skor Eczema Area Severity Index (EASI) hingga lebih dari 50%. Aktivitas multifaset ini menegaskan bahwa colloidal oatmeal tidak hanya berperan sebagai emolien, tetapi juga agen farmakologis yang menargetkan mekanisme molekuler untuk memperkuat pertahanan kulit, menurunkan inflamasi, dan mengurangi stres oksidatif. Temuan ini mendukung pengembangan formulasi topikal berbasis evidence-based pharmacology yang efektif untuk perbaikan skin barrier dan pengendalian inflamasi pada kulit sensitif, terutama pasien AD. Penelitian lebih lanjut diperlukan untuk mengevaluasi dosis optimal, stabilitas formulasi, dan efek jangka panjang pada populasi yang lebih luas.
